LICENSED PRACTICAL NURSE

Assessment and Care Planning
Our LPNs conduct initial and ongoing assessments of clients' physical and emotional needs. They develop and implement individualized care plans in collaboration with clients, families, and the healthcare teams.
Medication Administration
Our LPNs administer medications and treatments as healthcare professionals prescribe and monitor medication side effects, reporting any changes on time to the Physician.
Wound Care and Management
Our LPNs assess and manage wounds of all stages, including dressing changes and wound cleaning. They implement wound care protocols, while also providing education to clients and families
Communication and Collaboration
Our
LPNs communicate effectively with clients, families, and healthcare teams to ensure continuity of care. They collaborate with other healthcare professionals to develop and implement care plans.
Vital Signs and Health Monitoring
Our LPNs take and record vital signs, including temperature, pulse, respiration, and blood pressure. They also monitor and report any changes in the client's health status, including, but not limited to any signs of infection or deterioration.
Documentation and Record-Keeping
Our LPNs maintain accurate and up-to-date records of client care and progress, and document medication administration, vital signs, and other relevant information.
Education and Support
Our LPNs provide education and support to clients, families, and caregivers on health promotion, disease prevention, and management of chronic conditions, and encourage client’s independence and self-care.
Other Responsibilities
Our LPNs participate in quality improvement initiatives and continuing education to stay current with best practices, and perform other duties as required.
